FORM, A QUARTERLY JOURNAL CONTAINING POETRY, SKETCHES, ARTICLES   . . .  WITH PRINTS, WOODCUTS, LITHOGRAPHS, CALLIGRAPHY, DECORATIONS AND INITIALS. EDITED BY AUSTIN O. SPARE & FRANCIS MARSDEN. VOLUME ONE, NUMBER ONE. London: John Lane, April, 1916.  Atlas folio (sic), 58 pages. The sheets laid into the original lightly worn wrappers, a  clean copy of this fragile rarity.
Contributors to this issue included Laurence Housman, W. H. Davies, Laurence Binyon, Walter De La Mare, T. Sturge Moore, J. C. Squire, R. B. Cunninghame Graham & W. B. Yeats.  The artists included Frank Brangwyn, Charles Ricketts, Charles Shannon et al. Only two numbers were issued: April 1916 & April 1917.  It was resurrected after the War by the Morland Press (prospectus laid in) and once again was short lived, with only 3 numbers issued, 1921-22. Hoffman, "The Little Magazine",p. 249.
